Terms of Service

Effective Date: 11/02/2026

Welcome to HealthSource Medical Associates. HealthSource Medical Associates is a virtual medical practice providing telemedicine healthcare, medical evaluations, medical documentation, and related healthcare services. Services may be provided through live video visits, secure electronic communications, or asynchronous online requests when clinically appropriate and permitted by applicable law.

By accessing our website, patient portal, or services, you agree to these Terms of Service. Please read these Terms carefully before using our services.

1. Services Provided

HealthSource Medical Associates provides virtual healthcare and related medical services, which may include:

  • Virtual primary care and chronic disease management

  • Evaluation and treatment of eligible acute medical conditions

  • Medication management and eligible prescription refills

  • Doctor’s notes and medical certificates

  • FMLA evaluations and certifications

  • Short-term disability evaluations and certifications

  • ADA accommodation evaluations and supporting documentation

  • Travel-related medical documentation

  • Emotional Support Animal (ESA) evaluations and letters when clinically appropriate

  • Veteran independent medical opinion and related medical-record review services

  • Other medical evaluations, documentation, and healthcare services offered through our platform

The availability and appropriateness of any service depend on the patient’s clinical circumstances, location, applicable law, provider licensure, and the independent professional judgment of the evaluating healthcare provider.

Submission of a request, completion of an intake form, payment for a service, or participation in a consultation does not guarantee that a prescription, medical certification, letter, accommodation recommendation, disability certification, or other requested documentation will be issued.

2. Consultations and Evaluation Requirements

Depending on the service requested, applicable law, and the patient’s clinical circumstances, an evaluation may be conducted through a live video visit, asynchronous online assessment, secure electronic communication, or another legally permitted telemedicine method.

Certain services, including FMLA, short-term disability, ADA accommodation, virtual primary care, and other services requiring more comprehensive clinical evaluation, may require a live video consultation.

A healthcare provider may determine that additional information, medical records, testing, an additional consultation, specialist evaluation, or an in-person examination is necessary before a diagnosis, treatment recommendation, prescription, or medical certification can appropriately be provided.

Patients are responsible for providing accurate and complete information and for participating in any evaluation reasonably necessary for the requested service.

3. Payment and Insurance

HealthSource Medical Associates operates primarily as a self-pay, fee-for-service practice. Unless otherwise expressly stated, payment is due at the time a service is requested or scheduled.

HealthSource Medical Associates does not guarantee insurance coverage or reimbursement for any service. Upon request, we may provide a superbill or other documentation that patients may submit to their insurance carrier for possible out-of-network reimbursement. Eligibility for reimbursement and the amount reimbursed are determined solely by the patient’s insurance plan and carrier.

Prices for services may change from time to time. The applicable price will generally be displayed before the patient purchases or schedules the service.

4. Availability and Hours of Operation

Current operating hours and service availability are posted on our website and may vary by service, provider availability, patient location, and applicable law.

HealthSource Medical Associates reserves the right to modify operating hours, appointment availability, service offerings, and processing times as necessary.

5. User Responsibilities

  • You must provide accurate and complete information when using our services, including but not limited to, medical history, symptoms, and other relevant details.
  • You are responsible for ensuring that you meet the eligibility criteria for the services provided. For prescription refills, a valid prescription history with a healthcare provider is required.
  • You agree to comply with all applicable laws, including those related to telemedicine and online consultations.
  • By using the platform, you acknowledge that some services, such as prescriptions, are only available for stable and chronic conditions. For urgent or life-threatening conditions, you should seek in-person medical care immediately.
  • You agree to provide truthful, accurate, current, and complete information, including information regarding your identity, medical history, symptoms, medications, prior treatment, employment or disability circumstances when relevant, and any records submitted for review.
  • You may not alter, falsify, misrepresent, or misuse medical documentation issued by HealthSource Medical Associates. Medical documentation reflects the independent clinical judgment of the issuing healthcare provider and may not be modified or represented as supporting information that the provider did not certify.

6. Medical Documentation and Third-Party Decisions

Medical certifications, doctor’s notes, disability documentation, accommodation recommendations, travel-related documentation, independent medical opinions, and other documents are issued only when supported by the healthcare provider’s independent professional judgment and the information reasonably available at the time of evaluation.

HealthSource Medical Associates does not guarantee that an employer, school, insurance company, disability administrator, government agency, airline, Veterans Affairs, or other third party will accept, approve, or act upon documentation provided by HealthSource Medical Associates.

Any final determination regarding eligibility for employment leave, disability benefits, workplace accommodations, insurance benefits, reimbursement, government benefits, or other third-party programs is made by the applicable employer, administrator, insurer, agency, or other decision-maker.
