Welcome to HealthSource Medical Associates, a telemedicine service platform that provides healthcare services to individuals for prescription medication refills, doctor’s notes, virtual primary care, and other healthcare services related to chronic, stable, and acute conditions. By using our services, you agree to comply with the following Terms of Service. Please read these terms carefully.

1. Services Provided

Our platform offers the following services:

  • Prescription Medication Refills for Chronic and Stable Conditions: We provide refill services for ongoing medication management for stable, chronic conditions. Patients must have been previously prescribed the medication by a licensed healthcare provider.
  • Doctor’s Excuse Notes for Mild Acute Illnesses: We issue doctor’s notes for mild acute illnesses (e.g., colds, flu) to excuse you from work or school for up to 5 validated days within a 30-day period. These absences can be retroactively validated for up to 5 days before the date of submission.
  • Customized Notes for Unique Situations: In some cases, we provide customized doctor’s notes for unique situations that may require a video visit with a healthcare provider.
  • Acute Care Asynchronous Treatments: We offer asynchronous treatments for various acute illnesses, including but not limited to, minor infections, rashes, and other non-lifethreatening conditions.
  • Virtual Primary Care: Our platform provides virtual primary care visits, including the ability to order lab tests, imaging, and referrals for specialized care.
  • FMLA Certification: We offer Family and Medical Leave Act (FMLA) certification upon proper evaluation.
  • Short-Term Disability and ADA Certification: We issue short-term disability application certifications and Americans with Disabilities Act (ADA) certification for employees upon appropriate consultation

2. Video Visits and Consultation Requirements

  • For services like FMLA certification, short-term disability application, ADA certification, and virtual primary care, a 15-minute video visit with a licensed healthcare provider is required.
  • Video consultations are conducted through our secure online platform, which allows you to meet with a doctor remotely. You must ensure that your equipment meets the technical requirements for the video visit.
  • You are responsible for providing accurate medical information during the consultation, and the healthcare provider may recommend additional testing, treatments, or referrals based on your consultation.

3. Payment and Insurance Policy

  • Our platform operates on a self-pay We do not accept insurance, and services must be paid for at the time of service.
  • Payment for services can be made through the payment methods provided on the platform.

4. Availability and Hours of Operation

  • Our services are available Monday through Friday, closed on Saturdays.
  • We reserve the right to adjust our hours of operation at any time, and we will notify users of any significant changes to our operating hours.

5. User Responsibilities

  • You must provide accurate and complete information when using our services, including but not limited to, medical history, symptoms, and other relevant details.
  • You are responsible for ensuring that you meet the eligibility criteria for the services provided. For prescription refills, a valid prescription history with a healthcare provider is required.
  • You agree to comply with all applicable laws, including those related to telemedicine and online consultations.
  • By using the platform, you acknowledge that some services, such as prescriptions, are only available for stable and chronic conditions. For urgent or life-threatening conditions, you should seek in-person medical care immediately.

6. Privacy and Confidentiality

  • Your privacy is important to us. Our platform complies with applicable laws, including the Health Insurance Portability and Accountability Act (HIPAA), to protect your health information.
  • By using our services, you consent to the collection, use, and disclosure of your personal health information as described in our Privacy Policy, which can be found on our website.

7. Limitations of Services

  • Our platform provides telemedicine consultations for non-emergency medical services. For emergencies, please call 911 or visit the nearest emergency room.
  • We do not provide prescriptions for controlled substances or for conditions that require in-person evaluation.
  • While our platform offers healthcare services and advice, it is not a substitute for inperson medical care when necessary.

8. Cancellation and Refund Policy

  • If you need to cancel a scheduled consultation, please do so at least 24 hours in advance. Cancellations made within 24 hours of the scheduled appointment may not be refunded.
  • Refunds for services are issued at the sole discretion of HealthSource Medical Associates, depending on the circumstances.

9. Indemnification and Limitation of Liability

  • By using our services, you agree to indemnify and hold harmless HealthSource Medical Associates, its employees, assistants, contractors, and affiliates from any claims, damages, or liabilities arising from your use of the platform.
  • We do not guarantee the outcome of any treatment or service provided through our platform. You acknowledge that medical treatment involves inherent risks, and results may vary from person to person.
  • We are not liable for any delays or failures in the provision of services due to technical issues, system outages, or other factors beyond our control.

10. Termination of Service

We reserve the right to suspend or terminate your access to our platform at any time, with or without cause, including violations of these Terms of Service.

11. Changes to Terms of Service

  • We may modify or update these Terms of Service at any time. Any changes will be posted on this page, and the updated terms will be effective as of the date of posting.
